The proposition
A public health reporting tool built during the COVID-19 vaccine rollout. Members of the public record which vaccine they received and how they are feeling afterwards, and the severity of their answer determines how much the app asks for. Mild reactions are captured in a single tap; serious ones escalate into a structured adverse-event report. The framing is civic rather than clinical: "Support the warriors by providing your valuable feedback."
Design direction
Reassuring and non-clinical. A teal and cyan brand with generous white space, friendly flat illustration of health workers, and photorealistic 3D renders of vaccine vials, a syringe, and virus particles on the splash. The severity choice is expressed as three colour-coded faces rather than medical terminology, keeping a potentially anxious moment approachable.
What was designed
- Phone-number login with a four-digit SMS verification step and a resend affordance
- Vaccine selection, choosing the dose received from a dropdown of named vaccines
- Severity triage, a three-way choice presented as green, amber, and red faces labelled Minor, Severe, and Serious
- Symptom checklist for the Severe path, covering fever or chills, cough, muscle or body aches, and headache
- Full patient intake for the Serious path, a multi-step form progressing across five stages and capturing name, address, country, city, state, zip, email, and phone
- Confirmation state, an illustrated thank-you modal closing the loop on submission
What it demonstrates
A clean example of progressive disclosure driven by risk: the app asks almost nothing of a user with mild symptoms and escalates to a full structured report only when the answer warrants it. Also shows sensitive-context UX, turning a medical reporting task into something a nervous member of the public will actually complete.
